The secret door leads to another Optional Boss (troglodyte) and a shrine. If you don't have the optional witch doctor quest you may have an optional quest to kill a named wolf in front of the door to Iss'lar.

To get to Iss'lar you activate the crystal then you can go to the other side (either by walking or swiming underwater to the room) and activate the second crystal. Then jump in the water and go to the newly unlocked underwater area.

Some swimming is needed so if you aren't very good at it get an underwater item or potion.
