Enter the quest and follow the librarian around till he finds the secret wall. Once it opens a ghoul will attack your party. Regardless of how fast he's dispatched the librarian will die and you move on after grabbing the fragment.
Move along the hall to #2 on the map and obtain the next fragment. Move along the hall killing anything in your path until you reach #3 on the map. If you have someone who can search for secret doors, do so here. Behind this door is an Arcane Skeleton (rough guy) so be prepared as you reveal the door. Grab the chest and move on to #4.
You'll find another secret door, a few large brown spiders and a Black Widow that lies behind the secret door. Once you've dispatched them all, grab the fragment and move on towards #5 on the map.
You'll find another secret door here that reveals more undead and the final fragment piece. Grab it and climb down the ladders to hit the bottom floor. Move along the bottom floor killing undead and spiders until you have dispatched every monster, then exit out.
